It is strongly urged that all medication be administered to children at home. If prescription medication must be given at school, a medication form must be completed by the parent/guardian and physician, or a prescription form signed by the physician attached to the medication form. All medication must be transported to and from school by the parent/guardian in its original labeled prescription bottle.
