Health Concerns List |
Reminder..... Health Concerns List need to be picked up in the health room as soon as possible. Health Concerns list will be available in the nurse’s office during the first week of school. Please stop by the health room to sign for your list. The Health Concerns list is provided to you to keep you informed of any medical/health issues, as well as accommodations, if needed. Please keep your health concerns list in a safe and confidential place. I suggest that you highlight students names who are in your class. I will collect them at the end of the school year for shredding.

Emergency Health Care Plans (EHCP) |
Please be aware that students who require special accommodations due to a medical condition will have an Emergency/Individualized health care plan. You will be asked to attend a brief in-service to review the EHCP. Please keep this document in a confidential place. A copy of the plan will need to be placed in your sub folder. |

Flu Clinic scheduled for October 28, 2010. Faculty & staff-6:30am-7:30am. Students from 7:30-until completed. Clinic will take place in room 1.

Peanut/Tree nut Information |
Please assign one desk/table/chair to be peanut/tree nut free in your classroom. Place the provided sign on the desk and chair. Students should not be allowed to eat any food containing peanut/tree nuts at this desk. If you have a student who requires sitting at this designated desk, you will need to clean the desk with spitfire before the class begins. Supplies will be provided. |

Field Trip Information |
Please review the field trip guidelines that are in your Teacher’s Handbook.
Please note that the guidelines are issued from the Town of Lincoln and must be followed. It is my responsibility to ensure the safety of all students attending the field trip.
Item 12 in the outline states that one-week advance the teacher/teachers in charge of the field trip should review each child’s returned permission slip. (Please note that Lincoln High School’s permission slips are 2-sided.) If a slip is marked yes for medication needed, the permission slip needs to be copied (both sides) and given to me for review. Along with all permission slips marked yes, a copy of the field trip list of students attending need to be submitted (submit via email to Kathy Murray). I will then review the list and forms and return a completed list of students with medical issues or the need for medication on the trip. This list is to accompany you on the field trip. (Please note, this is confidential information and should be kept in a safe place.)
There have been many instances where the parent will mark yes on the permission slip stating that my office has information on file and we do not. I then must call the parent and have the proper documentation sent from the Doctor. Contacting parents and Doctors can be time consuming and a difficult process. This is why a minimum of one week is required.
Please Note: The information on the list that you will receive from my office will contain information that is on record. Due to some medical issues being left off the permission slip, please take this list with you.
